| Changes to Diabetes Education Program |

The Certified Nurse Diabetes Educator for the South Bruce Grey Health Centre’s Walkerton, Durham and Chesley sites has left the organization to join the Brockton & Area Family Health Team. As part of the Ontario Government’s push toward a Family Health Team model for primary care, funding for diabetes education is being directed to the Family Health Team and the Grey Bruce Diabetes Outreach Program.
SBGHC is working with the Family Health Team and the Grey Bruce Diabetes Outreach Program to define its future role in diabetes education. During this transition period, SBGHC has proceeded to contract the services of a nurse educator from the Grey Bruce Diabetes Outreach Program. Unfortunately, it will take time to recruit this position.
In the immediate future, persons currently attending the Diabetes Education Program at SBGHC will continue to be followed by dieticians associated with the program. The program may change in the future in that persons attending the hospital may find themselves referred to the Brockton & Area Family Health Team for service.
SBGHC encourages those patients with access to the Brockton & Area Family Health Team to contact their office for information on their diabetes education program. Those patients who do not have access to the Family Health Team, or do not have a family doctor, can contact SBGHC directly.
